MN Senior Care Partners is a Geriatric Care Management company helping seniors, adults with disabilities and their families navigate the complex elder healthcare system in Minnesota. Kristin Smith has over 20 years of non-profit / social service experience. She has assisted hundreds of families in Minnesota over the course of her career.
Kristin can help families with an initial assessment to identify the main issues / concerns surrounding an older loved one. Then a care plan will be created and guidance given to provide the family with an appropriate plan of action. Additional support services can be provided to find home care agencies, identify the right housing options, figure out how to pay for services and find supplemental assistance to support not only the senior but the family as well.
All services are provided on an hourly basis and work is only performed as needed/requested by the family and client. Kristin can work with a family for as little as an hour up to providing ongoing consultation, support as needed.
MN Senior Care Partners services the Twin Cities area of Minneapolis and St. Paul including the 9 county area: Hennepin, Ramsey, Washington, Wright, Sherburne, Dakota, Carver, Scott and Anoka.
