The SummitRidge Center for Psychiatry & Addiction Medicine provides mental health and substance abuse treatment services. Established in 1993, it supports a range of inpatient, partial hospitalization and outpatient programs that help adolescents, adults, senior citizens and their families. The center provides a variety of online forms, including patient registration, authorization for the release of confidential information, consent to routine procedures and treatments, medical screening, the notice of privacy practices and certification of patients legal representative. Located in Lawrenceville, Ga., the SummitRidge Center for Psychiatry & Addiction Medicine administers the Adult Partial Hospitalization Program, which is a treatment option available for people 18 years or older who have been diagnosed with a psychiatric condition or an addiction.
